Exposition à l’acide valproïque in utero et troubles neuro-psychologiques : la grande étude de l’ANSM (Frédéric Haroche, Journal International de Médecine)

Paris, le lundi 25 juin 2018 – L’ANSM (Agence nationale de sécurité du médicament) publie les résultats de son enquête visant à estimer le risque de troubles neuro-développementaux chez les enfants exposés in utero à l’acide valproïque prescrite à leur mère pour le traitement d’une épilepsie, étude basée sur les données du Système National des Données de Santé (SNDS, anciennement SNIIRAM).
Les enfants nés entre le 1er janvier 2011 et le 31 décembre 2014, sans diagnostic de malformation cérébrale à la naissance et dont la mère était affiliée au Régime Général de l’Assurance Maladie ont été inclus et les données les concernant analysées jusqu’au 31 décembre 2016, soit 1 721 990 sujets dont 8 848 avaient été exposés in utero à un traitement antiépileptique en monothérapie, dont 991 exposés à l’acide valproïque.

4 à 5 fois plus de troubles mentaux et du comportement

Les résultats mettent en évidence un risque marqué de troubles neuro-développementaux précoces en lien avec l’exposition à l’acide valproïque. En effet, à un âge moyen de 3,6 ans, parmi les enfants exposés, le risque d’avoir eu un diagnostic de « troubles mentaux et du comportement » lors d’une hospitalisation ou pour une reconnaissance d’affection de longue durée apparaît de l’ordre de 4 à 5 fois plus élevé que parmi les enfants dont la mère n’a pas reçu ce type d’antiépileptique pendant la grossesse.
Cette augmentation de risque concerne plus spécifiquement les « troubles envahissants du développement » (troubles du spectre autistique), de « retard mental » et de «troubles du développement psychologique » (troubles des fonctions cognitives et de leurs acquisitions). De plus, le risque de recours à un orthophoniste apparaît de l’ordre de deux fois plus élevé. Ces augmentations de risque persistent en l’absence de maladie psychiatrique maternelle. De plus, le risque de troubles neuro-développementaux précoces augmente avec la dose d’exposition à l’acide valproïque. Ainsi, le risque de diagnostic de troubles mentaux et du comportement apparaît jusqu’à 8 à 10 fois plus élevé parmi les enfants exposés aux doses les plus fortes comparés aux enfants non exposés, et le risque de recours à un orthophoniste jusqu’à plus de trois fois plus élevé. En outre, parmi les enfants exposés aux doses les plus fortes le risque de recours à un orthoptiste apparaît presque deux fois plus élevé que parmi les enfants non exposés.

Des donnés sur les dangers en fonction du trimestre de grossesse

Jusqu’alors les études disponibles ne permettaient pas d’établir si le risque différait selon la période d’exposition au cours de la grossesse. En effet, les différents travaux publiés demeuraient contradictoires et si certaines études suggéraient que l’augmentation du risque de troubles neurodéveloppementaux survenait préférentiellement en cas d’exposition tardive au cours de la grossesse, d’autres ont rapporté des effets néfastes quelle que soit la période d’exposition pendant la grossesse.
Les résultats de l’enquête de l’ANSM, permettent, eux, de conclure que la période d’exposition à risque de troubles neuro-développementaux pourrait se situer plus particulièrement au cours du deuxième et/ou du troisième trimestre de la grossesse. Rappelons, cependant, qu’il est clairement établi qu’une exposition à l’acide valproïque durant le premier trimestre de grossesse expose à un risque augmenté de malformations congénitales majeures.

Un risque beaucoup moins marqué, voir nul, avec les autres antiépileptiques

Ces travaux se sont aussi intéressés aux enfants exposés, in utero, aux autres antiépileptiques.
Il apparait que le risque de diagnostic de troubles mentaux et du comportement parmi les enfants exposés in utero à la lamotrigine (principale alternative à l’acide valproïque) est 3 fois moins élevé comparé aux enfants exposés à l’acide valproïque. L’exposition à la lamotrigine apparaît cependant associée à un risque global de diagnostic de troubles mentaux et du comportement augmenté de 1,6 fois, avec en particulier une augmentation du risque de « retard mental » et de « troubles du développement psychologique »…mais ces associations ne persistent pas lorsque l’analyse est restreinte aux enfants nés de mère sans maladie psychiatrique identifiée.
L’exposition à la prégabaline apparaît associée à un risque global de diagnostic de troubles mentaux et du comportement augmenté de 1,5 fois, avec en particulier une augmentation du risque de « retard mental » (mais de façon non statistiquement significative) et de recours à un orthoptiste. L’association avec le risque de retard mental se renforce et celle avec le recours à un orthoptiste persiste en l’absence de maladie psychiatrique maternelle.
L’exposition à la carbamazépine et l’exposition au phénobarbital apparaissent associées à un risque augmenté de diagnostic de certains troubles mentaux et du comportement, mais pour la carbamazépine ces associations ne persistent pas lorsque l’analyse est restreinte aux enfants nés de mère sans maladie psychiatrique identifiée, et pour le phénobarbital ce résultat ne repose que sur un seul cas parmi les enfants exposés.
L’exposition au lévétiracétam, à l’oxcarbazépine ou au topiramate apparait associée à un risque augmenté de recours aux soins (en orthoptie pour le lévétiracétam et l’oxcarbazépine, en orthophonie pour le topiramate et en psychiatrie pour le lévétiracétam). Cependant, aucune association avec un risque augmenté de diagnostic de troubles mentaux et du comportement n’a été retrouvée pour ces médicaments, et pour l’oxcarbazépine et le topiramate ces associations ne persistent pas lorsque l’analyse est restreinte aux enfants nés de mère sans maladie psychiatrique identifiée. Enfin, l’exposition in utero au clonazepam ou à la gabapentine n’apparait pas associée au risque de troubles neuro-développementaux.
Soulignons, néanmoins, comme le fait dans un communiqué le Comité national épilepsie, qui s’indigne de la décision d’interdire, presque totalement, cette molécule aux femmes en âge de procréer que certains patientes « ne peuvent pas se passer du valproate » et que la substitution n’est pas toujours possible.

Quelques faiblesses

Si cette étude vient confirmer et amplifier d’autres résultats publiés dans la littérature, ces travaux présentent néanmoins quelques faiblesses signalées par les auteurs eux-mêmes, des biais qui pourraient contribuer à une sous-estimation des risques.
Parallèlement, la morbidité psychiatrique maternelle, qui est parfois la cause de troubles neuro-développementaux chez l’enfant, n’a pu être prise en compte que partiellement du fait de l’absence d’informations détaillées sur la pathologie psychiatrique maternelle dans les données du SNDS. Aussi, si ce manque d’information a conduit à ne pas analyser dans cette étude l’acide valproïque utilisé dans les troubles psychiatriques, il est à noter que la morbidité psychiatrique maternelle est généralement plus fréquente chez les mères traitées par antiépileptique que chez les mères non traitées.
Enfin, d’autres facteurs de confusion potentiels telles les caractéristiques de l’épilepsie maternelle ou les QI parentaux n’ont pas pu être pris en compte car ils ne sont pas renseignés dans le SNDS.

Le nombre d’enfants difficile à évaluer

Du fait des différentes limites énoncées précédemment, les données fournies par cette étude « ne peuvent être utilisées pour établir une estimation du nombre absolu de cas de troubles neuro-développementaux parmi les enfants exposés in utero à l’acide valproïque en France » souligne l’ANSM qui estime néanmoins qu’en appliquant « le taux d’incidence cumulée de troubles neuro-développementaux parmi les enfants exposés rapporté dans la littérature, soit 30 à 40%, il ressort que le nombre total d’enfants atteints après une exposition dans l’indication épilepsie pourrait atteindre entre 13 200 (estimation basse) et 26 500 (estimation haute) ».
En somme, cette étude intéressante, qui vient confirmer des données observées précédentes et apporter quelques éléments nouveaux, souffre des limites propres aux études « Big data ».
